I will contact you with an appointment reminder the day before we are scheduled to meet. It is very important that, should something arise, you please notify me within 24 hours to cancel or change your appontment. You can call, email or text me directly in order to reschedule. Should you not cancel or are a no-show to a scheduled appontment, you will be charged a fee of $50. In the case of chronic issues with attendance, I reserve the right to terminate treatment.
HIPAA (Health Insurance Portability and Accountability Act of 1996) protects the relationship between a client and therapist. Your personal health information (PHI) cannot be disclosed without your written permission. This includes diagnosis, notes, treatment plans and bills. I am required to maintain the privacy of your PHI per HIPAA regulations. Exceptions to this include:

You are expected to pay your co-pay or treatment fee in either cash or check at the time of service, unless alternative arrangements have been made. I will be happy to provide you with a receipt. Fees are based on services requested. Should a financial concern arise before or during treatment, please discuss it with me and we can work on a solution.
Situations arise wherein treatment with me does not offer the appropiate level of necessary care. Should medication, substance abuse treatment or hospitalization be required, I will refer you to the appropriate facility and or physician.
